Repeat headgaskets and egr cooler

2007 F-450 6.0 was at another dealer 11 months ago and 9000 miles ago. That dealer performed 09-08-03, replacing head gaskets, oil cooler, egr cooler, cleaned turbo, flushed cooling system. Vehicle now comes into our shop overheating and running rough. Diasassembled vehicle, found head gasket leaking at cylinders #2 and #4 and egr cooler leaking. checked flatness of cylinder head and block, both ok. Any suggestions on what may have been the cause of this besides turbo and/or EBP sensor? Thought it was odd that both the head gasket and egr cooler failed.

Is there an oil cooler kit (base #6A642) on the repair history? I've seen a couple that guys managed to replace egr cooler and not oil cooler kit without Ford catching it and kicking it that have had repeat failures, got one at work now...another 'tech' in my store replaced the egr cooler 4000 miles ago (under warranty) without replacing the oil cooler, now the coolant is full of oil and the guy has no warranty....

Does it appear that all parts were actually replaced?

 

 

Any DTC's for overheating, fan clutch, etc? Are there any aftermarket components/modifications that might give a clue (snowplow, PTO, etc)? Is there any restriction in the radiator/cooling module from tree fluff, leaves, bugs, etc? Was the proper coolant in the system? Can you remove the WP and verify the impeller is tight and there is no cavitation damage behind the pump? Is the floor mat worn out under the pedal from WOT? Posted Image

Well, put it all back together and did notice EOT more 22 degrees more than ECT. Replaced oil cooler, looks much better. So either the oil cooler just didnt last long or wasnt replaced during the last repair like they said.
